Someone finally decides to ask for help, finds an addiction treatment center online, and dials the number. What happens next depends almost entirely on who, or what, picks up the phone. We wanted to know what really happens when you call an addiction treatment center after hours, so we called.
Key findings
-
In an October 2026 overnight test of 62 addiction treatment centers, 13 of the 16 calls with a clear result never reached a person.
-
7 went to voicemail, 4 rang with no answer, and 2 got a busy signal. 3 may have reached a person.
-
In 11 follow-up calls placed by hand the next morning, 0 reached a person.
-
46 overnight calls hit phone menus or gave unclear results and were excluded rather than counted as missed.
What did the overnight test find?
Most overnight calls to addiction treatment centers that produced a clear result never reached a person. Of 16 calls with a verifiable outcome, 13 ended in voicemail, no answer, or a busy signal. Only 3 may have reached a live person.

The test covered 62 addiction treatment centers. The remaining 46 calls reached phone menus or produced inconclusive results, so we left them out of the count rather than guess what happened.
What happened on the follow-up calls the next morning?
None of the 11 follow-up calls reached a person. Dave Sichel, founder of Blueshirt Media, placed these calls himself, one at a time, the following morning. Some went to addiction treatment centers already included in the overnight test.

How was the test run, and what are its limits?
This was a practical field test, not a scientific survey of the industry. The overnight calls were placed and classified automatically. Calls that reached a phone menu were set aside for manual verification instead of being counted as missed, and calls the system couldn’t classify were marked inconclusive.
The numbers above report only what could be confirmed. An addiction treatment center that routed callers through a phone menu to a live person would not show up as a miss here.
The follow-up calls were a separate check, made by hand.
Why do unanswered after-hours calls matter for addiction treatment admissions?
An unanswered call often ends the conversation for good. In a September 2025 CallRail survey of 1,000 U.S. consumers, 78% said they had abandoned a business after an unanswered call. Only 42% said they leave a voicemail, and 21% said they immediately call another business.
That survey covered businesses in general. People calling an addiction treatment center are in a different position. Many are deciding, in that moment, whether to get help at all. That moment of willingness can be short, and it rarely waits for a callback the next business day.
There is also a cost to the addiction treatment center. Centers pay for the clicks, ads, and listings that produce those calls. When the phone goes unanswered, that spend produces nothing, and the person who called may end up somewhere else, or nowhere.
Why do addiction treatment centers miss calls overnight?
Missed overnight calls are usually a coverage problem, not a staff problem. Admissions teams generally work business hours. Overnight clinical and nursing staff are caring for patients and can’t stop every time the phone rings. Answering services, where they exist, may handle many clients at once.
The result shows up later in the numbers. Census comes in below plan. Leadership asks why. Marketing points to the calls it generated, admissions points to the calls it never got, and both are right. The calls went to voicemail.
How can an addiction treatment center test its own after-hours phone line?
You can run the same test on your own addiction treatment center tonight in about five minutes.

If you reach a person who can answer admissions questions, your coverage is working. If you reach voicemail, a menu without your name, or silence, that is what callers hear too.
What should a caller hear when they call an addiction treatment center after hours?
A caller should reach a real conversation, not a recording. Good after-hours coverage for an addiction treatment center looks like this:
-
The call is answered quickly, every time, including nights, weekends, and holidays
-
The greeting names your center, so the caller knows they reached the right place
-
Common questions about insurance, levels of care, and next steps get accurate answers
-
Callers ready to talk to admissions are connected to a live person when one is available
-
When admissions is unavailable, the caller gets a scheduled callback instead of voicemail
-
Anyone in crisis is escalated immediately according to the center’s protocol
What does HIPAA require for after-hours call answering?
Any service that answers admissions calls for an addiction treatment center handles protected health information and must meet HIPAA requirements before the first call is answered.
-
Business Associate Agreement (BAA): signed before onboarding, not after go-live
-
Encryption: call recordings, transcripts, and caller data encrypted at rest and in transit
-
Audit trail: a record of who accessed caller information and when
-
Crisis escalation: a defined, tested protocol for callers at risk, routed to live staff or emergency resources
-
42 CFR Part 2: substance use disorder records carry additional federal confidentiality protections beyond HIPAA
If a vendor can’t show you each of these in writing, it isn’t ready to answer your phones. For callers who need immediate help, the SAMHSA National Helpline is free, confidential, and available 24/7.
Questions to Ask Before You Choose After-Hours Call Coverage
-
Will every call be answered, including nights, weekends, and holidays?
-
Does the greeting use our center’s name?
-
Is the service built for behavioral health, or adapted from a general answering service?
-
Will you sign a BAA before onboarding?
-
How are recordings and transcripts encrypted and stored?
-
How are callers in crisis identified and escalated?
-
How does a caller get connected to our admissions team, and what happens if nobody is available?
-
Does call information flow into our CRM so admissions sees every lead the next morning?

Frequently Asked Questions
What happens when you call an addiction treatment center after hours?
In an October 2026 test of 62 addiction treatment centers, 13 of the 16 overnight calls with a clear result never reached a person. Most went to voicemail, rang with no answer, or got a busy signal. Results vary by center, so the best way to know is to call your own line after hours.
How many calls to addiction treatment centers go unanswered at night?
No industry-wide figure exists. In Blueshirt Media’s October 2026 test, 13 of 16 overnight calls with a clear result never reached a person, and 0 of 11 follow-up calls the next morning reached a person. The test was a field check, not a scientific survey.
Do people leave a voicemail when a business doesn’t answer?
Many don’t. In a 2025 CallRail survey of 1,000 U.S. consumers, only 42% said they leave a voicemail when a business doesn’t answer, and 21% said they immediately call another business.
Why do addiction treatment centers miss calls at night?
Most admissions teams work business hours, and overnight staff are caring for patients. Without dedicated after-hours coverage, calls fall to voicemail, phone menus, or an answering service handling many clients at once.
Is AI call answering HIPAA-compliant for addiction treatment centers?
It can be, if the vendor signs a BAA before onboarding, encrypts data at rest and in transit, keeps an audit trail, and has a defined crisis escalation protocol. Substance use disorder information also falls under 42 CFR Part 2, which adds confidentiality requirements.
How can I test my addiction treatment center’s after-hours phone coverage?
Call your public admissions number after hours from a phone your staff won’t recognize. Count the rings, listen for your center’s name in the greeting, and note whether you reach a person, voicemail, a phone menu, or nothing.
